An approach to predicting the course of diabetic polyneuropathy in patients with type II diabetes mellitus has been developed and validated, based on a comprehensive assessment of TNF-alpha levels and nerve conduction parameters. Threshold values for TNF-alpha levels (>12 pg/ml) have been established, in combination with reduced sensory (SNCP <20 m/s, SNAP <2 mV) and motor (PMNAP <3.2 mV) conduction parameters, characteristic of the painful form of diabetic polyneuropathy. Correlational relationships were identified between TNF-alpha levels and key electroneuromyographic parameters, enabling their use to assess disease severity and predict its course. The rationale for the use of complex therapy incorporating alpha-lipoic acid, acetyl-L-carnitine, zinc, nervonic acid, blueberry extract, B vitamins and selenium has been established, as this helps to reduce TNF-alpha levels and improve patients’ clinical condition.
