Aim was determination of peculiarities and diagnostic value of clinical symptoms, heart structural and functional remodeling, heart rate variability in coronary CHF patients and creating of estimation algorithm of coronary CHF risk stratification. Methods: Standard clinical examination, analysis of irradiation influence, 24-hour blood pressure monitoring, ECG monitoring, heart rate variability monitoring, dopplerechocardiography, bicycle ergometry. Results: peculiarities and diagnostic value of clinical symptoms, heart structural and functional remodeling, heart rate variability in Chernobyl disaster survivors with coronary CHF were determined. Based on receiving data algorithm of coronary CHF risk stratification was created. It was discovered that high risk of coronary CHF was characterized on dispnoe, tiring, symmetric feet oedema. High sensitive and specific of left ventricle hypertrophy, variants and grade of left ventricle dysfunction, index of myocardium mass, relative left ventricle wall posterior thickness, ratio of left atrium size to end-diastolic size, end-diastolic and end-systolic volume, ejection fraction, IVRT, E/A ratio for coronary CHF were discovered. It was determined that normal ECG in CHF patients was absent. High sensitive and specific of rest ECG ST-depression and high grade ventricular ectopy for CHF was determining. It was shown that heart rate variability have sufficient role determining of stage, functional class, estimate of risk of CHF progressing. Regular exaggerating of low frequency and very low frequency according relative normal total power spectrum of heart rate variability as well as sympathetic activation on beginning stage of CHF was discovered. Regular lowering of exercise tolerance and physical working capacity in coronary CHF patients was shown. Informative criteria that characterizing high and low risk of coronary CHF in Chernobyl disaster survivors as well as CHF progressing predictors were selected. Estimation algorithm of grade of CHF risk and management of CHF patients was created. Efficacy: Receiving data and created algorithm of coronary CHF risk stratification have sufficient value for minimization of CHF influence on health and capacity Chernobyl disaster survivors in long-term period after irradiation.
