The aim of study is to determinate VDJ rearrangement of B-CLL patients, who didn't suffer due to Chornobyl NPP catastrophe. The collection of RNA and DNA samples from irradiated B-CLL patients. Methods of study are polymerase chain reaction with following sequencing of PCR products, immunophenotyping of peripheral blood cells, immunoenzyme methos for serum Ig determination, clinico-hematological, statistical methods. The structure of CDJ rearrangement of heavy chain immunoglobulin's genes in B-CLL (chronic lymphoid leukemia) patients, inhabitants of Ukraine, which did not have direct influence of ionizing radiation, have been analyzed. The prevalence of unmutated cases (69% of all patients) and VH1-69 gene (32,5%) was revealed. Another genes of variable fragments of immunoglobulins included VH3-09, VH3-30, VH3-33 in combination with D2-2 or D3-9, and JH6 genes. Association of VDJ rearrangement with serological sings of viral infection of TORCH group did not found. The significance of mutation status of immunoglobulin's genes for B-CLL prognosis have been revealed. The increasing number of advanced cases over the follow-up period, shorter median time from diagnosis to start of second line therapy, worse response to fludarabine treatment, poor survival for early stages B-CLL were found in unmutated vs. mutated CLL patients.
