Aim: to determine of the clinical course features, morphological and molecular disturbance in CMPD patients with different apoptotic potential suffered from Chornobyl accident. Methods of the study: clinical, morphological, histological, cytogenetic, molecular, immunological and mathematical. Results: CML course basing on tyrosinkinase treatment general status and blood quantitative indices was normalized but the multiply qualitative changes in granulocytes among suffered from Chornobyl accident preserved. The cell differentiated disturbance in megakaryocytes lineage lead to increase the functioning immature bone marrow was determine. In OMF patients suffered from Chornobyl accident the aggregation function of platelets was found what it need to application of the individual therapeutic approach to resolve thrombosis and hemorrhagic complication. Cytogenetic examination in CML patients Ph- chromosome in all group without othersdeviation of karyotype and clonality was confirmed. Molecular and genetic examination in CML patients especially among suffered from Chornobyl accident transcript b3a2 (94,5%) found. In CML patients from opposite group the presence of transcript b3a2 with molecular mass 210 kDа and b2a2 (190 kDа) in ratio 1:1 determined. Transcript coexpression b3a2 and e1a2 in CML cases can showed one of the prognostic criteria about clonality development of diseases. Treated by BCR/ABL-tyrosin inhibitors the CML patients in chronic phase on the different stages of diseases not lead to significant increasing CD95 positive lymphocytes and expression of bcl-2 protein. In granulocytes of CML patients treated by tyrosin kinase inhibitors the level of Fas-receptor was increased what it showed for availability BCR-ABL-positive cells to be in programm cell death status but the increase intracellular level of Bcl-2 protein formed the block to implementation of this process. The increasing procent of р53-positive cellswas determined in both lymphocytes and granulocytes population of cells in CML patients with partial remission compare with complete remission. The pathogenetic background of diagnostics was justified, course prognostics and therapeutic approach development accounting the complex of clinical and haematological features CML course of the apoptosis progress and molecular-genetic disturbance on different time of diseases was showed.
