Primary and current control during hospital treatment is conducted for all patients, protracted supporting therapy at 48 from them. Goal: The determination of clinical displays, changes of indexes of Holter electrocardiography monitoring, structural and functional state of myocardium, tolerance to physical exercise in clean-up workers after Chernobyl disaster with most widespread heart arrhythmias, the creating of programs on their correction. Methods: clinical, analysis of irradiation influence as well as others risk factors, Holter electrocardiography and heart rate variability monitoring (HEM), dopplerechocardiography, bicycle ergometry. Results: At primary control clinical changes and changes of most indexes of HEM, dopplerechocardiography, bicycle ergometry in CWCD with extrasystolia and AF are certain. The most meaningful changes were at extrasystolia more than 1000 for days and paroxysmal AF. Number of ventricular extrasystoles correlated with duration of myocardium ischemia, left ventriculus wall thickness and myocardium mass index. Taking into account these information the program of treatment of extrasystolia and model of treatment of PF is developed. In 4 week of hospital treatment for certain the amount of extrasystoles diminished in 2,4 time, pair and group ventricular extrasystoles - in 2 times, supraventricular extrasystoles - in 3,2 time. Frequency of episodes of myocardial ischemia diminished on 12,2 %, duration of episodes of general and silent myocardial ischemia on 1 patient - on 5,4 and 11,9 mins, ischemia at the physical loading - on 7,7 mines. There was an improvement of diastolic function and increase of ejection fraction on 12,2 %, to threshold power to 50-100 W. At continuation of supporting therapy during a year an effect increased for ventricular extrasystoles in 1,2 time, supraventricular extrasystoles - in 1,4 time, pair and group extrasystoles - in 5,7 time. Duration of a 1 episode of ischemia diminished in 1,4 time (P<0,05). There was stabilizing of dopplerechocardiography indexes, tolerance to physical exercise was increased. For patients with AF supporting therapy on the developed model assisted renewal of rhythm, warning or diminishing of frequency and duration of episodes AF, stabilizing of heart rate at level less 70 in 1 minute, stabilizing of dopplerechocardiography indexes, to diminishing of functional class chronic cardiac failure. Efficacy. Application of the program with the protracted supporting treatment extrasystolia and models of treatment AF confirmed their efficiency for optimization of treatment CWCD with these HA on condition of adherence of patients to treatment.
