Research (work) purpose - to improve the effectiveness of multimodality therapy and the quality of patients life with the diagnosis of the urinary bladder invasive cancer by means of using neo-adjuvant chemotherapy and research of its influence on anatomical - histological structure of the urinary bladder tumour and also the improvement of surgical (tactics) of treatment. Research methods: clinical, laboratory, X-ray - radiological, endoscopic, instrumental, pathomorphological, statistical. It has been established that the implementation of neo-adjuvant poly-chemotherapy during the treatment of patients with the invasive urinary bladder cancer diagnosis allowed to get complete recovery in 9,1 % cases, and in 50 % - partial regression of tumorous process. Analysis of the treatment results which has been conducted among the patients with the diagnosis of the invasive cancer of urinary bladder has exposed a directly proportional relation between the level of tumour differentiation (G) and the level of tumour regression under the effect of the neo-adjuvant poly-chemotherapy (G1 - 87,5 ± 17,7 %; G2 - 52,7 ± 29,3%; G3 - 23,1 ± 43,4% according to RECIST criteria). Conducting of the neo-adjuvant poly-chemotherapy among the patients who suffer the urinary bladder invasive cancer allowed to improve or normalize kidneys function and uro-dynamics of the upper urinary tracts in 40%. An authentic prostate decrease in size, improvement of urination act and quality of life in general were detected as a result after the conducting of the neo-adjuvant poly-chemotherapy among the patients with invasive CUB (cancer of the urinary bladder). It has been found out that the neo-adjuvant poly-chemotherapy causes a regression of the invasive CUB in 59,1 % and decrease of invasion depth in 54,5 % which gives the possibility to conduct a surgical intervention which would be of "organs preserving" type and thereby get the patient's quality of life much more better. It was discovered that there is a direct dependence of the increase of apoptosis index from the tumour regression level under the effect of the neo-adjuvant poly-chemotherapy according RECIST (р < 0,05) criteria which allows to recommend the usage of imuno-fluorescent investigation with the using of confocal microscopy as an effectiveness of poly-chemotherapy prognosis criteria to among the invasive CUB patients. It was indicated that the injection of dexketoprophen 20-22 hours before the surgery procedure helps to get a clear postsurgical anesthesia as compared to the using of analgesics on an early preoperative stage (р=0,001). Besides, it is proved that the usage of dexketoprophen 20-22 hours before the beginning of the surgery leads to the decrease of Е2 prostaglandines level in plasma and CSF (Cerebro - Spinal Fluid) as compared to the patients who took placebo and ketorolak (р=0,0009 and 0,0002), which signifies about a decrease of central sensitization and pain chronization in a further postsurgical period.
