Object of study - 356 gynecologic oncology patients (80 - cervical cancer patients, 34 - cancer of the corpus uteri, 205 - cancer of the corpus uteri, 12 - cancer of the vulva, 25 - trophoblastic disease of pregnancy) which from 2008 to 2010 in the Gynecologic Oncology Department of the National Cancer Institute were operated due to cancer of the female reproductive system. Objective: To improve treatment gynecologic oncology patients through the development and optimization of surgical techniques to improve exposure and requirements for organ, combined and extended surgery. The assessment of the immediate results of applying methods in patients with initial cervical cancer has been shown that recurrence of disease were absent in 100% patients within 2 years after treatment. Reproductive function and social adaptation have been preserved in all patients, moreover, the sexual function also was restored. Preserving treatment in patients with malignant tumour of the ovary stage IA has allowed to keep the reproductive function in young women: in all patients maintained menstrual function. Anterior exenteration pelvis in patients with generalized malignancy of the cervix is the only radical method of treatment for patients with disseminated tumour to the bladder. The effectiveness of this method could be assessed only after studying the immediate and remote results treatment of these patients. Methods of retroperitoneal panhysterectomy and pelvic perytonectomy (Douglasectomy) which were introduced into clinical treatment practice of patients with disseminated malignant ovarian tumours increases the radical operations to 30%. Application of combined surgical interventions in patients with generalized cancer of the vulva is an efficient and effective treatment, resection of annexes does not significantly affect the duration of surgery and blood loss, postoperative complications, besides old age is not a reason for refusal in surgical treatment. It was established linkage between the expression level of p53, Ki-67, ER, PR and main morphological characteristics of the tumour (depth of invasion and degree of differentiation) in patients with endometrial cancer. T These preliminary results confirms high potential of this research: implementation into clinical practice of radical preserving surgery prevented the development postcastration syndrome and preserves the reproductive function of women that was impossible with standard treatment оncogynecological patients, improve the quality of life of patients. The effectiveness of treatment gynecologic oncology patients with new surgical techniques can fully appreciate only after studying and evaluating long-term results.
