RA was diagnosed in 21 patients, 6 patients had RAEB and another 2 - were CMML patients. Aim of the study was to determine factors that could contribute to development of non-effective hemopoiesis in MDS and stimulate clonal evolution and disease transformation into acute leukemia. General clinical and specific hematological investigations were performed. The results of studies performed were used to determine three risk groups for potential transformation into acute leukemia. Chromosome aberrations were found in 7 patients, their incidence was higher in RAEB patients as compared to the patients with RA. Patients with MDS were revealed with different degree of deviation from normal ranges of proapoptotic (TNF-?, TGF-?) and proangiogenic (VEGF) cytokines. Their level depended on the disease subtype, bone marrow cellularity, severity of anemia, level of platelet counts and presence of signs of transformation. Level of TNF-? in patients with RA and RAEB was higher in those subjects with greater severity of anemia, leucopenia and thrombocytopenia whereas VEGF level had direct correlation with hemoglobin levels in both subtypes of MDS. The obtained results suggest that cytopenic syndrome may be caused by influence of TNF-?. Such a suggestion, however, can not be referred to the other analyzed cytokine, belonging to the group of proapoptotic agents, namely TGF-?1. One can not exclude that in patients with MDS more expressed is the ability of this cytokine to stimulate tumor clone proliferation which together with angiogenesis activation causes disease progression.
