Description
Abstract
Objective. To determine the effect of the proposed method of stimulation of the diversion loop before closing the
protective ileostomy on the incidence of postoperative dynamic intestinal obstruction.
Materials and methods. The study included 37 patients who underwent examination and treatment in the tumor
surgery department of the Transcarpathian Antitumor Center between 2020 and 2025. Patients were randomized into
two groups. The control group included 20 patients who did not undergo stimulation of the diversion loop before
ileostomy closure surgery. The study group included 17 patients who underwent stimulation of the diversion loop
before ileostomy closure.
Results. In the control group, ileostomy was closed on average in (142 ± 45.1) days, in the study group – in (132 ± 35.2)
days. The difference between the groups in terms of ileostomy closure time was not statistically significant (p = 0.62).
In the study group, the incidence of dynamic intestinal obstruction was 1.7 times lower. The difference between the
groups in terms of this indicator was statistically significant (p = 0.047). The first bowel movement after surgery in the
control group occurred within 3 to 7 days, on average (4.4 ± 1.23) days. In the study group, the first bowel movement
occurred earlier – within 2 to 5 days, on average (3.35 ± 1.11) days. The difference between the groups in this indicator
was statistically significant (p=0.012). The length of stay in the hospital after surgery for patients in the control group
ranged from 7 to 19 days, with an average of (10.95 ± 4.07) days; patients in the study group were discharged 1.54 days
earlier, with an average of (9.41 ± 2.53) days (p0.05).
Conclusions. Preoperative irrigation stimulation of the diversion loop is a sufficiently effective method for preventing
acute dynamic intestinal obstruction after protective ileostomy closure in patients with rectal cancer.
