Objects of research: chronic obstructive pulmonary disease (COPD), bronchial asthma (BA), pneumonia. The purpose of the research: to study new mechanisms of the formation, course and progression of COPD, BA and pneumonia, the pathogenic well-foundation of the possibility of the diseases course control, the improvement of the methods of diagnosis, prophylactic and patient treatment. The methods of the research: questionnaires, clinical, laboratories, instrumental, functional, microbiology, bacteriology, immunology, morphology, statistical. Results of the research. It was done the long-term clinical-functional monitoring of COPD and BA patients; clinical and functional features of the course of the diseases were studied. The new pathogenic mechanisms of the formation, course and progression of COPD due to local and systemic inflammation, mucociliary transport, endothelial dysfunction, state of the protheinase inhibitory system were revealed. The risk group of the COPD patients due to high temp of the disease progression was established. The clinical-functional and gender features of the BA course were defined, the risk factors of the formation of generalization parodontitis were established. The schemes and methods of the prophylactic and treatment of COPD and BA patients were developed and improved. The clinical-anamnesis and microbiology features of non-hospital and hospital pneumonias were studied, the indications for antibiotic therapy were improved. Novelty of the results. The system of long-term treatment, monitoring and medical rehabilitation of the COPD and BA patients was developed, certified and introduced. The methods of diagnosis, prophylactic and treatment of COPD, BA and pneumonia patients were well-founded, developed and improved. Efficiency of the innovation: prevention of the severe exacerbation of COPD and BA, decrease of the number of hospitalization, prevention of the formation of severe complication of the diseases, improvement of the methods of diagnosis, treatment and prophylactic. Sphere of the innovation: polyclinic, therapeutic and pulmonology departments.
