Objects: patients with rectal cancer, malignant breast tumors, and skin melanoma. Aim: To improve the diagnostics of regional lymph node metastases and optimize the surgical treatment in patients with rectal cancer, breast cancer and skin melanoma. Material and methods. 70 patients with rectal cancer, 100 patients with breast malignancies and 51 patients with malignant melanoma were included in study. Diagnostics: sonography, endoscopy, dermatoscopy, histology, oncomarkers, and statistical methods. Treatment: resection of rectum, "lateral" lymph node dissection, breast sparing operations, and sentinel node biopsy in melanoma patients. Results. Integrated ultrasonic inspection of the primary tumor and regional lymphatic collectors is a highly informative noninvasive technique which in 75-92 % of cases can establish metastatic lymph nodes even in preoperative period in patients with rectal cancer, breast cancer and skin melanoma. Improved preoperative ultrasound diagnosis of pelvic metastatic lymph nodes in patients with rectal cancer allows to carried out the resection of rectum with precise "lateral" lymph node dissection, and thereby avoid the likelihood of relapse in the remote postoperative period. Ultrasound diagnosis of the primary tumor and regional lymph nodes in patients with breast cancer stage T1-T2 can be objective criteria for organ sparing operations, and control of local changes in the breast and lymphatic collectors during patient's follow up. Breast sparing operation should be carried out at sufficiently developed gland, when the ratio of tumor: breast more than 1:75-100. Ultrasound "in vivo" thickness and volume of skin melanoma coupled with dermatoscopy and cytology are objective criteria for the wide local excision of the primary malignant melanoma. Ultrasonic monitoring of regional lymphatic collectors in patients with skin melanoma is an effective way of early diagnosis of metastatic lymph nodes, and the criterion for selecting patients for performing regional lymphadenectomy.
