Objective: to improve the diagnostics of JRA based on studying the clinical polymorphism of the disease, metabolic disorders and genetic factors. Methods: clinical, instrumental, immunological, biochemical, genetic, and statistical. Results: the study made it possible to establish the signs for early diagnosis of JRA and its differential diagnosis regarding reactive arthritis. Our study has confirmed a steady progressive course of JRA, which is characterized by the presence of microcirculatory changes, retaining of T-lymphocytes low activity and an increase in IL-6, redistribution of glycosaminoglycans as well as collagen catabolism, and a high degree (72%) of the genetic load. Gender differences in the clinical manifestations of the JRA long-term course have been defined. Four variants of the disease clinical course have been singled out and the model of their development with the informativity of 86.2%, sensitivity 76.8% and specificity 75.0% has been designed. Novelty: our study has established current regularities in the JRA development at the early stages and in the duration of the disease up to ten years on the background of the treatment, including the evolution of the articular syndrome, changes in the microcirculation, cellular and humoral immunity, cytokine status, proteoglycans and collagen metabolism, and chromosome apparatus stability. A novel algorithm for early diagnosis of JRA and its differential diagnosis in comparison with reactive arthritis has been developed. A long-term catamnesis enabled the authors to single out four variants of the JRA remote consequences: remission for at least two years, stabilization of the process on the background of minimal activity, recurrence without further development, and progression of the disease symptoms. A predictive model of the variants development has been worked out as well. Effectiveness: early diagnosis of the JRA, considering a long-term character of its course, increase the effectiveness of treatment by 75%.
