Comparison of the clinical course, cardiohemodynamic parameters, structural and functional changes of the heart in patients after myocardial infarction with comorbid hypertension, depending on the presence of heart failure and left ventricular diastolic function, has been carried out.It is clarified that in patients with chronic heart failure of ischemic genesis, a moderate decrease in GFR is associated with a significant increase in galectin-3, ST2, cystatin C with the presence of close positive correlation relationships. It was first established that in patients with CHF of ischemic genesis with normal renal function, there were found strong direct correlation relationships between galectin-3, ST2 and parameters of structural remodeling of the left ventricle of the heart. Scientific data showing that GDF-15, NTproBNP and E/E had the most prognostic potential for the occurrence of unfavorable cardiovascular events in patients with heart failure with preserved left ventricular ejection fraction who had had myocardial infarction with comorbid hypertension were expanded. In patients with asymptomatic diastolic dysfunction who had MI on the background of hypertension, GDF-15 had the greatest prognostic significance for the occurrence of unfavorable cardiovascular events. Scientific data showing that the use of combination of GDF-15 and NTproBNP increase the positive predictive value of each of the biomarkers regarding the onset of unfavorable clinical consequences were clarified. Adding the E / E or left atrial volume index (LAVI) ratio to the combination of GDF-15 and NTproBNP resulted in further increasingof the positive predictive value of biomarkers concerning the onset of unfavorable clinical events in patients with HF PLVE who have had myocardial infarction with comorbid hypertension.
