Aim - to increase the effectiveness of treatment in patients with neutrophilic bronchial asthma and bronchial asthma combined with chronic obstructive pulmonary disease, taking into account their endotypes. Methods - questionnaires, clinical, functional, radiological, laboratory, statistical. The main clinical and functional features in patients with neutrophilic bronchial asthma (BA) and asthma combined with chronic obstructive pulmonary disease (COPD) have been established. Characteristic features of patients with these endotypes are uncontrolled course of the disease, the presence of small bronchi obstruction and fixed bronchoobstruction, mainly infectious-allergic nature of exacerbations, the treatment, requires the use of antibacterial therapy. The comorbidities are - allergic rhinitis, obesity, cardiovascular diseases, arthritis, symptoms of mild to moderate depression and sleep-disordered breathing - are observed. There were worked out technologies: application of ultrafine glucocorticosteroid in basic therapy of patients with BA with neutrophilic type of inflammation, which allows to achieve clinical and functional efficiency in 80,0 % of patients; use of combined drug budesonide / formoterol in basic therapy in steroid-naive patients with BA combined with COPD with neutrophilic inflammation, which allows to achieve clinical and functional efficacy in 93,3 % of patients. A method of treating patients with BA with neutrophilic inflammation has been developed, the use of which allows to improve asthma symptoms control, reduce the frequency of exacerbations, improve bronchial patency at the level of small bronchi, reduce fixed bronchial obstruction, increase exercise tolerance, that results in functional efficiency in 93,3 % of patients.
