The object of study - clinical, molecular, genetic characteristics of patients with plasma cell myeloma (PCM). Objective: to determine the range of polymorphic variants of cytokine genes and genes of the main histocompatibility complex as prognostic criteria for risk of occurrence and course of PCM in persons suffered after the Chornobyl disaster and persons with not burdened radiation history on the basis of comparison with population distribution and their association with pathological process. Research methods - clinical, molecular, genetic, statistical. A complex study of polymorphism of cytokines genes (TNF-α, TGF-β1, IL-6, IL-10, IFN-γ) and HLA genes (HLA-A, HLA-B, HLA-C, HLA-DRB1, HLA-DQA1, HLA-DQB1)) has been conducted to predict the risk of occurrence and course of plasma cell myeloma in the long term after the Chornobyl accident. 102 patients were examined, including 56 victims of the Chornobyl accident. There was established dependence of the predictive effect on the state of the mutant IL-10 allele: (Exp (β) less than 1 (0.579)) for IL-10-1082 A/G (heterozygotes) - the protective effect was determined; association with the risk of disease - (Exp (β) = 2.85) for IL-10-1082 G/G (minor genotype) was revealed. The study of the contribution of multigenic combinations of the interaction "gene-gene" and "gene-environmental factor" shows their important role in the mechanisms of origin and course of PСM: the composition of markers HLA-B*52, C*06, DRB1*11 has a higher degree of association with the inability to achieve remission compared to isolated alleles of HLA-A*24, HLA-B*52, HLA-DRB1*11, which confirms the hypothesis of the presence of additive interaction "gene-gene" and clarifies mechanisms of associative connection with the development of the pathological process.
