Objective: To continue monitoring the incidence of thyroid cancer in children and adolescents of Ukraine affected by the Chornobyl fallouts, to compare the frequency of BRAFV600E mutations, invasive properties and postoperative follow-up in radiogenic and sporadic PTC, as well as in BRAFV600E-positive and BRAF-negative patients of different ages. Methods: morphological, immunohistochemical, statistical. Theoretical and practical results: The incidence of thyroid cancer per 100 thousand relevant population in high-risk groups (children and adolescents at the time of the Chornobyl accident), both in Ukraine as a whole and in the six most affected northern regions was characterized by increasing time-trends for the period from 1990 to 2020. The difference between the indicators in the least and most affected regions persisted until 2020, which may indicate the presence of an additional radiogenic component of the disease in the contaminated regions of Ukraine for 34 years. The BRAFV600E mutation was more common in sporadic than radiogenic papillary thyroid carcinomas (PTCs), but the presence of such a mutation is not associated with higher tumor aggressiveness and worse prognosis in patients aged up to 28 years at the time of surgery, regardless of PTC's etiology.
