Purpose: based on retrospective and current analysis of the bronchopulmonary system in the remote post-accident period to scientifically substantiate measures to optimize the diagnosis and prediction of disabling diseases in persons exposed to ionizing radiation. Research methods: generally clinical examination, functional lung tests, X-ray and CT of the chest, videoobronchoscopy, cytological examination of bronchoalveolar lavage, statistics. According to the results of a retrospective assessment of the state of the liquidators at the Chernobyl, the prevalence of disabling bronchopulmonary system diseases is high (26%). The main forms are COPD (20.6%), bronchial asthma (5.4%), pulmonary fibrosis (0.1%). According to the results of a comprehensive analysis, the phenotypes of accelerated and slow progression of the bronchopulmonary system diseases of the victims of the Chernobyl acciden were identified. Signs of the presence and progression of disabling bronchopulmonary diseases are the risk factors (smoking, occupational hazards, frequent exacerbations, liquidators of the Chernobyl in the first three months, high BMI, several comorbidities, shortness of breath, low functional pulmonary tests and the rapid dynamics of their reduction). An algorithm for optimizing the diagnosis and treatment of disabling diseases of the bronchopulmonary system in persons affected by the Chernobyl accident, which consists in identifying high-risk groups in the early stages of disease, comprehensive clinical, functional, endoscopic and radiological examination, management of controlled risk factors for bronchopulmonary diseases and comprehensive treatment to reduce socio-economic losses of society.
