Purpose: to study the mechanisms of optimization of bone regeneration depending on the recipient age under the conditions of using allogeneic bone implants in combination with MSCs and bioactive factors. Results: It has been proven that the use of allografts for plastic surgery of large bone defects that do not heal spontaneously is promising. It has been established that allogeneic MSCs together with an allograft are impractical to use for the reconstruction of bone defects in the case of fresh injuries and without the use of osteoinduction of cells or immunosuppressants. The optimal mode of centrifugation for obtaining a GFBP was determined. It has been proven that the saturation of the allografts with GFBP or their local introduction in 7 days after the operation contributes to the complete replacement of the allograft with bone tissue, regardless of the age of the recipient. Novelty: New scientific knowledge was obtained about the mechanisms of bone regeneration in recipients of different ages under the conditions of reconstruction of large defects with bone allografts along or with GFBP or MSCs. For the first time, the optimal timing of the introduction of GFBP to optimize reparative osteogenesis and accelerate allograft remodeling has been determined. Effectiveness of implementation: the obtained results should be taken into account during the planning of further researchers and the development of treatment tactics for patients with massive bone destruction after high-energy injuries (mine-explosive and gunshot wounds), removal of tumors and major operations on the spine and joints.
