CHRONIC SUBDURAL HEMATOMA, CLINIC, COURSE,
NEUROIMAGING, SURGICAL TREATMENT, CONSERVATIVE
TREATMENT, DECLINED AND OLD AGE, MORPHOLOGY.
Research object: chronic subdural hematomas (HSG).
Research methods: clinical - neurological, instrumental, morphological, statistical.
Purpose: to improve the quality of diagnosis, differentiated tactics of surgical and conservative treatment of chronic subdural hematomas in elderly and senile patients.
The results. 166 people of different age groups were treated. Unilateral HSGs were found in 80% of cases, bilateral non-traumatic genesis were mostly found in people over 60 years old. A clinical and diagnostic characteristic of HCG in the elderly and senile is the presence of hemorrhage with a volume of ≥ 100 cm3 with a discrepancy between the severity of the condition, clinical and neurological manifestations, and the degree of expressiveness of dislocation-compression changes with the data of MSCT/MRI studies. Displacement of median structures according to MSCT, MRI up to 3 - 5 mm was observed in 22% of cases, 6 - 10 mm- in 42%, 11 - 15 mm - in 30%, and more than 15 mm - in 6%. Removal of the hematoma through milling holes with drainage is the method of choice for surgical treatment of HCG, which reduces the risk of recurrence of HCG. Drainage after surgical removal of HSG is not associated with an increased risk of infectious complications, but reduces the risk of recurrence of HSG. Microcraniotomy is recommended for hematoma with a complex neuroimaging architectonics (multiple septa, layers, trabeculae), in some cases of recurrence of hematomas.
