Goal: Defining the clinical structure of chronic ischemic heart disease (CHD), the prerequisites for its development, clinical, electrocardiographic, structural and functional myocardial changes, and exercise tolerance in CWCD consequences with angina pectoris to support new approaches. Methods: clinical, analysis of radiation exposure, other risk factors, blood pressure monitoring, daily monitoring of electrocardiogram (ECG DM), heart rate variability, the echodopplerechocardiography, bicycle ergometry. Results: The development of angina pectoris among the participants was preceded by the interaction of violations of forestry, acquired as a result of the Chernobyl disaster, with genetic and other factors, non-radiation nature. Priority had a job in 1986, the external exposure dose > 25 sZv, watch mode. Factors of non-radiation nature installed in all participants of forestry, mainly 2-3. More than 70 % had arterial hypertension (AH), high cholesterol, overweight, half - the habit of smoking, heredity, age - 45-55 years. Number of deaths from coronary heart disease and hypertension in 2,8 times higher than the number of deaths from cancer and 10 times from injury and poisoning. The percentage of deaths of working age with the duration of CHD up to 5 years was high. The most significant contribution to the structure associated with CHD belongs to the state AH. In 95,6 % of cases in CWCD was a chest pain. The frequency of angina attacks they have significantly exceeded the control data, 2 times more the frequency of use of nitroglycerin. Episodes of myocardial ischemia by ECG DM characterized by greater frequency, duration and number of 1 patient. Statistical and spectral indexes of heart rate variability and parasympathetic activity decreased. Hypertrophy of the left ventricle of the heart was registered in 93,6 % of patients, diastolic dysfunction of the left ventricle of the heart - at 70,5 %. After hospital treatment significantly decreased to 82,3 % frequency of angina attacks, at 81,8 % - the use of nitroglycerin, episodes of myocardial ischemia in 1,7 times, the average duration of episodes of myocardial ischemia - at 22,5 min. Increased parasympathetic activity and autonomic balance was improved. At 9,8 % reduction in myocardial mass index to 18 g/m2 (P <0.05), in 23,3 % improvement in diastolic function of the heart. Normal blood pressure achieved in 49,4 %, total cholesterol - 7,7 %. Significantly higher was threshold power of 10,8 W, the volume of exercise - 7,3 kJ. These results justify the long-term treatment of CWCD with angina pectoris, the adaptation of treatment to the patient, expanding the boundaries of the impact of factors of cardiovascular risk, early treatment.
