To define a complex of the most effective and cost-efficient diagnostic measures for early diagnosis of recurrences, regional and distant metastases of differentiated thyroid carcinoma during post-treatment monitoring. Research methods and equipment. Clinical, laboratory (thyroglobulin and antibodies content), radionuclide study, ultrasonic scanning, computed tomography, X-ray study, histological study. US equipment, gamma chamber, X-ray unit, immunoenzymometric (ELISA) analyzer, computed tomography scanner. Theoretical and practical results. The implemented program of post-treatment monitoring of patients with differentiated thyroid carcinoma included a diagnostic scheme of early detection of recurrences by a gradual estimation of stimulated levels of thyroglobulin and thyroglobulin antibodies, ultrasound investigation of the neck, X-ray study of the lungs, scintigraphy with 99mTc-(V)DMSA and, if necessary, CT of the thoracic organs and scintigraphy with radioiodine. The method of early diagnosis of differentiated thyroid carcinoma recurrence was elaborated. Pharmacoeconomic characteristics of immunometric and radiological methods of diagnosis of recurrences and metastases of differentiated thyroid carcinoma were provided, as well as efficacy of the post-treatment monitoring program for such patients was estimated. This factor allows a cost reduction for diagnostic complex by 18%. Novelty. For the first time the specificity (100%) and sensitivity (96.6%) of immunometric method of estimation of thyroglobulin and scintigraphy with 99mTc-(V)DMSA (sensitivity - 85%, specificity - 100%) were established regarding their ability to detect early the recurrences of differentiated thyroid carcinoma. Implementation effectiveness. Elaborated program of post-treatment monitoring of patients with thyroid cancer allows to reduce cost of diagnostic complex by 18%.
