Subject of the study – etiopathogenetic mechanisms of complication development in the acute and intermediate periods of combat traumatic brain injury (cTBI) and their prevention. Relevance. Combat traumatic brain injury is a pressing problem in modern neurosurgery and military medicine. Complications arising in the acute and intermediate periods of traumatic brain disease develop as a result of the combination of primary injury and secondary pathogenetic processes, have a prolonged course, and negatively affect functional status, psychoemotional condition, and social adaptation of affected individuals. This determines the need to improve approaches to their early diagnosis and comprehensive prevention. Aim of the study – to investigate pathogenetic mechanisms of complication development during different periods of the course of cTBI and to develop a method for their early diagnosis and prevention. Materials and methods. A total of 165 combatants with closed traumatic brain injury of 2 days to 12 months’ duration were examined. Clinical and neurological, psychological, neuro-ophthalmological, otoneurological, neuroimaging, laboratory, and electrophysiological methods were applied. Results and conclusions. It was established that cTBI is characterized by more pronounced and prolonged secondary brain damage compared to civilian traumatic brain injury. The structure of complications is dominated by psychoemotional, cognitive, and cerebrospinal fluid dynamic disorders with a tendency toward progression. The proposed method of early diagnosis and comprehensive prevention contributes to a reduction in complication rates and improvement of treatment outcomes.
