In the basis of this work it was put the results of examination and treatment of 1376 patients with thyroid cancer, who have been operated in surgical department of V.P.Komissarenko's Institute of Endocrinology and Metabolism of Ukrainian Medical Academy during 1980-1999. Aim of investigation. To analise the experience of diagnostics and treatment of ukrainian patients with malignant thyroid tumors after Chernobyl accident and create the modern programm of their surgical treatment, which directed to increasing of oncological radicality and decreasing of level of postoperative complications. Methods of investigation and equipment. In the work we used diagnostic (ultrasonography, X-ray, cytology, pathology, radiology, biochemistry), clinic-epidemiological, statistical methods. For treatment of patients we used the surgical instruments for performing of neck operations ("Martin", Gemany), general aneastesy equipment, patient observation monitors, sonography apparatus, cytological and pathological laboratories. Theoretical and practical results: 1. It were analized the particuliarities of clinical behaviour of malignant thyroid neoplasms in patients of different age groups after Chernobyl accident. 2. It was created the diagnostic-currative alhoritm for patients with thyroid tumors, which have appeared after radiactive influence. 3. It were shown the surgical management and operative extention for thyroid and regional lymphatic collectors in patients with thyroid cancer. It was created the indications for second operations for recurrent cancer and after primary non-radical interventions. 4. It were analized the immediate and remote results of radical surgical treatment in patients with malignant thyroid neoplasms and were shown the factors which influenced on disease output. New: The new in our work is in the analisis of clinical trend character of thyroid cancer in patients after Chernobyl accident, creation of new tactic of patient management and technical methods of surgery. We post ulated the practical recommendations for surgical tactics and operative extention in patients suspected to thyroid cancer, patients with microcarcinomas, high-invasive cancer and metastatic involvement of regional lymphatic collectors. Effectiveness of new-coming: As a result of implementation of new ways of treatment we are waiting for decreasing number of postoperative complications after surgery for thyroid cancer, increasing oncological radicality of surgery, decreasing level of cancer recurrences and increasing the life terms of patients. The fields of usage: The results of this investigation can be used in clinical practice of oncoendocrinology for surgical treatment of patients with malignant thyroid neoplasms and have significance for valuation of character of thyroid cancer trend at the next years.
