The purpose of work: to study clinical structure and dynamics of therapeutically resistant depressive disorders, to secure{discharge} their clinical, neurophysiologycal, biochemical, clinico-psychologic predictors and to develop classification of factors of a therapeutic resistance in conditions of a modern pathomorposis of mental disorders. As a result of the work it was found that clinical, neurophysiologycal, biochemical predictors of therapeutic resistance depend on a stage of depressive disorder (initial, remote, final) and type of course of disease: "classical", "involutional", "heteronomos", "continuous". To factors therapeutic resistance concern: a chronic serious psychogenia, hysterical personal features, premanifestation period, heteronomos course, the remote and final stages of disease, dyssomnic syndrome in the remission, the increased serotonin concentration, the decreased melatonin level, absencefunctional brain hemisphere asymmetries.
