Objective - to increase the efficiency of radiotherapy of patients with advanced cervical cancer (ACC) by use of chemoradiomodifiers and interferons. Methods and instruments: clinical, laboratory, radiological, ultrasonic, dosimetric. ROCUS, simulator of a session of radiation - "SimViw" 3000 (Siemens, Germany), AGAT - VU, US-unit "Aloca" FLEXUS, a unit for magnetic-resonance investigation (МРI) "Magneton-Vision-Plus-System (Siemens), universal dosimeter PTW Freiburg "Unidose E centrifuge "ОPN-3 " with revo 2000 g, flow cytofluorimeter " FAScan " (BECTON DICKSON, USA) № 82582, 1991. Results: Role of clinical, anatomo-morphological and biological features of tumor in formation of personal prognosis of the treatment of ACC has been studied. Complex of necessary examinations and their sequence in ACC patients for determination of primary tumor site, its morphological and anatomical features, which one are indispensable for planning of respective course of chemoradiotherapy (CRT) and monitoring of the efficacy of treatment has been developed. It has been studied the dependence of sensitivity of advanced cervical carcinomas to cytotoxic effect on the content and ploidy of DNA tumor genomes and parameters of proliferative activity of tumors, definited by DNA flow cytofluorimetry. The obtained data correlate with findings of clinical and instrument examination of ACC patients. The developed methods of conservative treatment of ACC patients which anticipate simultaneous use of combination radiotherapy (CRT), cytostatics at radiomodifying doses and/ or interferon. It has been determined indications and contra-indications for usage of designed methods of CRT with taking into account specific features of tumor process and other clinically significant factors of prognosis of disease. The complex examination and radiation treatment on a background of chemoradiomodifiers and/or interferon of 114 ACC has been performed. The designed methods of CRT improve the immediate results: the objective response of tumor (complete + partial regression) in use of phtorpyrimides was increased by 26,7 %, hydroxisurea – by 25,8 % and in combination with interferon – by 27,8 % and 37,0 % respectively comparatively with control group, which received only CRT that evidence on their efficiency. There was no general toxicity after CRT, which reliably would exceed ІІ rate and would negatively effected life quality of patients in both groups. The investigation of parameters DNA-status of tumor clones and their proliferative activity opens prospects for individual approaches to CRT of ACC patients. The dynamics of the contents and ploidy of cell DNA can be additional criterion of the efficiency of treatment. Field of use: specialized oncologic institutions of Ukraine.
