The object of study- neurovascular compression syndromes of cranial nerve (NCS CN). Purpose - to improve results of the treatment of NCS CN based on the investigation of their etiology and pathogenesis. Methods of research- clinical, MRI, ENMG, laboratory tests, electronmicroscopy. Such syndromes like trigeminal and glossopharyngeal neuralgy, hemifacial spasm, Menier's syndrome, paroxysmal arterial hypertention are presents severe painfull syndromes and hyperactive dysfunction of VII, VIII, X cranial nerves that leads to professional and social disability. These syndromes persistent for a long time with deteriorations, recurrences and progression and have no established treatment modalities. During the research (based on 560 patients with NCS CN) the necessity of at least two factors' simultaneous existence for NCS CN development was established. These are: 1) vascular compression of the nerve root which provokes local ischemia and postischemic changes and 2) immunodepression, autoneurosensibilisation. Our treatment modalities were based on these factors of pathogenesis and included immunocorrection and microvascular decompression of CN in case of failed medical treatment. Surgery was performed with innovations based on microanatomical peculiarities established during research. Application of new diagnostic methods including intraoperative neuromonitoring and treatment modalities based on the established pathogenesis complemented by new surgical methodologies improved nearest treatment results up to 96,9% and long-term results up to 89,2%. The results of research are introduced to the work of Neurosurgical Departments of Kyev, Uzhgorod, Dnepropetrovsk, Charkiv.
