Goal: Defining of the clinical follow-up, electrocardiography, structural and functional myocardial changes, and exercise tolerance in CWCD with angina pectoris during 1-year treatment with new approaches using. Methods: clinical, analysis of radiation exposure, other risk factors, daily monitoring of electrocardiogram (ECG DM), heart rate variability, the echodopplerechocardiography, bicycle ergometry. Results: Approaches to adequate treatment of CWCD with angina pectoris based on total risk conception, combined treatment that improve prognosis and lower ratio and intensity of angina attacks were substantiated. Total risk stratification algorithm and model of angina treatment in CWCD were elaborated. There was established angina attacks ratio and nitroglycerin using significantly lowering during 1-year treatment provided high compliance. Antiischemic and antiarrhythmic effects based on daily monitoring of electrocardiogram were established during hospital as well as 1-year treatment. Quantity and duration of ST-depression > 1 mm episodes were reduced in two times as well as asymptom ischemic episodes in 2.5 times. Indexes of total heart rate variability were increased after treatment. Vegetative balance was improved as well as sympathetic influence was diminished. Global and local myocardial contractility improvement took placed using basal model of treatment combined with cardiomyocytoprotectors. During 1-year treatment antioxidant system state was improved. Exercise tolerance was significantly increased during permanent treatment. In CWCD with angina pectoris and irradiated dose >25 sZv results of treatment were worse. In cases of high compliance to health life style blood pressure was lower to normal level in 50,5 %, ratio of obesity in very high risk patients was lower in 13.1 %, high risk patients was lower in 3.7 %. Permanent using of lipid lowering diet and statins promoted to lowering of cholesterol in very high risk patients to 1.8 %, high risk patients was lower to 6.7 %. Stopping of tobacco smoking predominantly was in very high risk CWCD with angina pectoris. Opposite in non-regular treatment were worsening of clinical follow-up, electrocardiography, structural and functional myocardial indexes especially in very high risk patients. Efficacy: Determining of the adequate approaches to treatment of CWCD with angina pectoris its influence on ratio and duration of ischemic episodes, global and local myocardial contractility, left ventricle diastolic function, oxidative stress, exercise tolerance was substantiated long-term treatment, the adaptation of treatment to the patient, expanding the boundaries of the impact of factors of cardiovascular risk, early treatment.
