Purpose of the research: To improve the efficacy of synchronous chemoradiation therapy for non-small-cell lung cancer by means of working out new technologies of application of a high-energy photon radiation and anti-tumor drugs with radiosensibilizing effect based on determining the mechanisms of chemoradiation influence on induction of ceramide-mediated tumor apoptosis. Methods of research:: biochemical, electron microscopy, morphology, clinical instrumental, radionuclide, dosimetry, statistical. Results and their novelty: For the first time, experimental investigation of the mechanisms of radiation effect which can potentiate at ceramide synthesis, increase of the enzyme activity and synergism at sphingosin production, were determined. It was found out that combined action of high-energy photon radiation and Etoposid promoted increase of synthesis of pro-apoptosis sphyngolipids (ceramide, sphyngosin) in the tumor and activity of Zn2+-dependent sphingomyelinase in the blood serum against a background of apoptosis processes activation in the tumor. The efficacy of chemoradiation therapy with Etoposid or Cisplatin in accelerated fractianation mode was proven It was determined that in the majority of patients with non-small-cell lung cancer blood serum Zn2+-dependent sphingomyelinase activity increased 2.7 times by the end of chemoradiation therapy with Elapsed or Cisplatin against a background of 50% tumor regression. Social and economic significance: Improvement of chemoradiation therapy efficacy with Etoposid or Cisplatin of the patients with non-small-cell lung cancer.
