Object of study: 189 patients with arteriovenous malformation (AVM) of the brain. . Objective: To improve methods of endovascular operations based on the study of vascular malformations architectonics and functional state of afferent vessels of AVM of the brain to improve patients treatment. Methods: Standard survey data analysis of physical and neurological status, total cerebral angiography , computed tomography, magnetic resonance angiography of the brain. Results: patterns defined structure and functional state of AVM afferent vessel malformations.Found that angioarchitectonics and functional state of afferent vessels of AVM affects the choice of method of endovascular treatment. Based on the obtained data highlighted two types of vascular architectonics AVM: 1st type - arteriovenous malformation with arteriovenous shunting indirect, 2nd type - direct arteriovenous shunting. Given the features angioarchitectonics AVM in 42 (23.9%) patients achieved total exclusion of malformations in 69 (39.1%) - excluded from the bloodstream more than 80% of the malformation and in 66 (37.0%) performed embolization in volume less than 80%. As a result of research developed a way to get a liquid to exclude AVM from cerebral blood flow (Patent of Ukraine № 50352). Efficiency: expanded range of possibilities of endovascular operations for AVM of the brain using technical devices for delivery of embolic substance in the structure of AVM. Using the proposed division of AVM for angioarchitectonic differences and given the functional state of afferent vessels of AVM, improvement achieved its radical one-off, reduced patient length of stay in hospital an average of 3 ± 1,2 days.
