Study object - 171 patients with RCC Study aim - decrease invalidization level, improve efficacy of diagnostics and treatment of patients with kidney cancer by indications and operation technique in organ-sparing approaches. Methods: clinical, laboratory, statistical and imaging. It was found that hypertension for more than 10 years in 73% of cases lead to chronic kidney decease, dictating partial resection without ischemia (р < 0,05). It was proven that diameter and volume of functional parenchyma correlate significantly with GFR (р < 0,05), making those parameters important for choice of treatment in RCC. Kidney resection lead to decrease in kidney function by 10,9% (р < 0,05) at 3 months after operation. It was shown that ischemia significantly decreased blood loss from 397,7 ± 236,9 ml to 332,7 ± 162,3ml (р < 0,05) during resection. Ischemia under 20min results into decrease of total GFR to 15,4% three months after and on 5% a year after resection and GRF on the affected side was decreased on 29,1 % and 19,4 % respectively (р < 0,05). New nephrometry system was developed (N.C.I.U.- nephrometry). It is based on tumor location, size and percentage of remaining functional renal parenchyma. This system helped to optimize indications for resection, preserve kidney function and prevent development of CKD. In polar locations of the tumor and remaining functional parenchyma of more than 55% it is worth to perform resection (р < 0,0001). In medial locations and remaining functional parenchyma of less than 70% and maximal size of tumor 4 cm and more nephrectomy can be a surgery of choice. Fast track surgery in partial resection for RCC was developed and allowed to decrease postoperative hospital stay to 4,9 ± 2,3 days (р < 0,0005) with the same level of postoperative complications.
