Purpose - to increase the treatment efficiency and improve the quality of life of patients with locally advanced breast cancer by improving methods of neoadjuvant therapy. A new method of complex locally advanced breast cancer treatment using neoadjuvant chemotherapy with intra arterial infusion of cytostatics by CP scheme and systemic infusion of anthracyclines has developed. The existing method of complex locally advanced breast cancer treatment using of neoadjuvant chemotherapy by FAC scheme with mild hyperyhermia in axillary area was improved. The immediate treatment results of the National Cancer Institute 385 patients, who received complex treatment for breast cancer from 2012 to 2014 were analyzed. The effectiveness of the developed and improved techniques of locally advanced breast cancer complex treatment using neoadjuvant intra arterial chemotherapy, neoadjuvant chemotherapy with mild hyperyhermia, chemoradiotherapy with radiomodifiers and using standard anticancer neoadjuvant treatment techniques of different stages of the disease was evaluated. It is advisable to conduct at least 4 courses of systemic or modified neoadjuvant chemotherapy to obtain the objective clinical and morphological effect in the treatment of patients with locally advanced breast cancer. The obtained results substantiate the perspectives of neoadjuvant chemotherapy following algorithm to locally advanced breast cancer: stage IIB patients begin treatment with neoadjuvant chemotherapy using mild hyperyhermia. In the absence of objective tumor response to treatment it is advisable to use chemoradiotherapy with ftorpirymidines. Patients with IIIA stage of disease should begin treatment with neoadjuvant chemotherapy using mild hyperyhermia or intra arterial chemotherapy. If no effect it is appropriate to use of chemoradiotherapy with radiomodifiers. Patients with IIIb stage of disease start treatment with intra arterial chemotherapy, if no effect should be used chemoradiotherapy with ftorpirymidines. Obtained preliminary positive results of locally advanced breast cancer treatment using developed and improved techniques indicate their feasibility and advisability of continuing research.
