Purpose - improving treatment results of patients with skin melanoma by developing new treatment techniques combined with the introduction of lymphadenectomy "sentry" lymph node.Methods and apparatus - clinical, morphological, radiological method indication "sentry" lymph node, surgical treatment, combined method of treatment, methods of statistical analysis of results, a portable gamma lychilnyk «EuroProbe», producer of "Sanberra-Packard», odnofotonyy emission computer 'Books tomography "OFET», «Spirit DH».Developed and tested a new method of combined treatment of patients with melanoma skin with the introduction sentry lymph node lymphadenectomy.It is proved that the use of methods guard lymphadenectomy lymph nodes in patients with melanoma skin male leads to a significant increase rate 3-year disease-free survival rate at 49.6% (p = 0.0001); in patients with primary tumor thickness of more than 4.0 mm - 25.4% (p = 0.030); in the absence of surface ulceration of skin tumors - 13.1% (p = 0.016).It was found that melanoma thickness 1,01-2,00 mm associated with a statistically significant decrease in the frequency of lesions guard lymph nodes (p = 0.0001), and the thickness of the melanoma more than 4.0 mm - with an increase (p = 0.005 ). At the threshold of the primary tumor thickness Breslow 2,8 mm likelihood of metastases in the lymph nodes guard increased 6.2 times compared with lower (OR = 6,2; p = 0.0001).Guard lymphadenectomy lymph nodes leads to a further reduction in the incidence of disease progression in the form of regional metastases in lymph nodes twice (p = 0.028), thereby improving local control.The applied methodology combined treatment of patients with localized primary melanoma skin did not change the rate of 3-year overall survival, and achieved an increase of 9.0% rate 3-year disease-free survival was statistically unreliable (p = 0.056).Thickness 2,01-4,00 mm primary tumor causes increased risk of progression to 5 times (p = 0.03), presence of ulceration - 2.5 times (p = 0.022), guard-wide involvement of lymph nodes or spread of tumor ekstrakapsulyarne - at 14.7 times (p = 0.0014).
