Objective: to Develop a set of measures of effectiveness of treatment for glaucoma, by identifying new aspects of the involvement of melatonin in the pathogenesis of the disease, the study of micro-invasive methods of surgical treatment of glaucoma and mechanisms of formation of the surgically created pathway outflow of intraocular fluid after anti-glaucoma operations. Prophylactic use of melatonin before, during and after the introduction of the stress-factor - adrenaline - made it practically possible to normalize the outflow and production of aqueous humor. We studied the influence of melatonin on oxidative stress and endothelial dysfunction. It was shown that melatonin reduces the expression of free radical processes in glaucoma animals, reducing the level of malonic dialdehyde by 24% and raising the total antioxidant activity of blood plasma by 33.5%. Melatonin had an impact on the function of endothelium in experimental glaucoma, increasing blood levels of vasoactive marker of endothelial dysfunction of nitric oxide by 39%. Course application of the drug melatonin - Vita-melatonin reduced the severity of oxidative stress in patients with POAG, decreasing blood level of the marker of lipid peroxidation malonic dialdehyde 53%, and decreased the endothelial dysfunction by increasing the blood levels of a marker of the vasodilation of nitric oxide at 110%. The study of the dynamics of IOP in patients with POAG after applying the technique of trabeculectomy "ab interno" using the operating system "Trabectome", shows the reduction of IOP by 21.5% in the medium term follow-up (173,0 ± 17,2) days. The mechanism of the hypotensive action of this type of surgery is associated with microinvasive removal of trabecular tissue which are the primary cause of resistance to outflow of intraocular fluid in patients with primary open-angle glaucoma. Investigated the severity of PTS in patients with POAG after trabeculectomy "ab interno" using the operating system "Trabectome". We studied the efficacy of beta-therapy in the prevention of excessive scarring after the OP the AGO. UBM identified characteristics of OP after the AGO and their quantitative value in relapsed increase IOP.
