The combination of glomerulosclerosis and tubular atrophy in the foci of stromal fibrosis are characteristic for the age-related changes in kidney of older donors. Kidneys from donors older than 50 years without any clinical signs of systemic diseases and renal dysfunction demonstrated slight degree of glomerulosclerosis and renal arteriosclerosis and adequately resisted to ischemic and reperfusion injury, which allowed their usage for transplantation. Combined evaluation of urine sediment and cytokine profile of urine can help to clarify the cause of chronic renal allograft dysfunction Increasing concentrations of beta-2 microglobulin in serum may serve as a marker of glomerular dysfunction. Beta-2-microglobulin, AST and IL-2 are diagnostic biomarkers which allow to determine localization, and activity of the of the pathological process. In addition, they allow you to predict a low GFR, and the drop in GFR > 25% from basal level and rapid decline in GFR during 2 years of follow-up.
