Objective: To increase the effectiveness of the combined treatment of patients with ovarian cancer stage III - IV through the development of technology of individualization of neoadjuvant chemotherapy based on an assessment of molecular and biological properties of the tumor using videolaparoskopii. Research Methods and Accs: clinical, radiological, immunological, morphological, statistical. Theoretical and practical results: it is proved that the patients with ovarian cancer stage III - IV who received 3-4 courses NHT in 11.1% of cases had reached the full, and in 88.9% - a considerable tumor regression, which corresponds to III - IV degree of therapeutic pathomorphism. A significant reduction in mitotical index, on the one hand, and the increase in the number of pathological mitosises and index of apoptosis, on the other hand, indicates the loss of "aggressive" OC properties held under the influence of HT. The combination of a pronounced expression of VEGF and a high level of a tumor marker CA-125, in patients without a NHT may indicate highly metastatic tumor activity, which is a negative factor in ovarian cancer prognosis. Lack of expression of VEGF and Bcl-2, a high level mt p53 and Ki 67 is a high risk factor for recurrent ovarian cancer prognosis in a generalization of the process, which necessitates amplification of anticancer therapy. The novelty: for the first in Ukraine the individual sensitiveness of tumour is certain to the chemotherapy on the basis of research of therapeutic pathomorphism а chasse of ovaries of III-IV of the stage. First in Ukraine certain groups of patient with a cancer ovaries of III-IV of the stage depending on the type of relapse and level of molecular-biological markers of tumour.
