Purpose of the study: to increase the treatment efficiency of patients with cerebral arteriovenous malformation by introducing a new transarterial - transvenous approach. Methods of research: standard questionnaire, objective and neurological status, digital subtraction angiography, computer tomography, magnetic resonance imaging. On the basis of evaluation the result of treatment of 221 patients with AVM, dural arteriovenous fistulas, carotid-cavernous fistulas, features of venous drainage of various cerebral arteriovenous malformation and hemodynamic changes after endovascular treatment were investigated; the results of only endovascular treatment of arteriovenous malformation and its complications were studied; we introduced a new method of endovascular treatment of arteriovenous malformation using combined transarterial - transvenous approach; we evaluated a new method and its complications; possible ways of complications elimination or their minimization were found. On the basis of obtained results a new approach for the treatment of cerebral arteriovenous malformation was introduced, that allowed to increase the radicality of elimination and effectiveness of endovascular treatment. Endovascular interventions were performed in 208 (94.1%) patients, 13 (5.9%) patients were not operated (because of the absence of indications for surgical intervention, refusal of treatment), and radiosurgical treatment was recommended. In ten patients (4.6%) transvenous approach were utilized, eight of them had a combined transarterial - transvenous approaches. In one patient during the early postoperative period after transarterial - transvenous approach , massive Intraparenchymal- intraventricular hemorrhage has occurred with subsequent fatal outcome. We studied the aspects of the venous systems structure of the arteriovenous malformation and features of cerebral venous hemodynamics before and after surgical intervention. A new method for the treatment of arteriovenous malformation has been introduced (Patent No. 116394 UA, MPC (2017.01) A61B 17/00; A61F 2/00. The technology of patients treatment with cerebral arteriovenous malformation was developed. The implementation of the investigated findings into practice will improve the treatment efficiency of patients with arteriovenous malformation, increase surgical exclusion of arteriovenous malformation of surgical interventions by reducing the level of complications.
