The purpose of the work is to increase the level of medical and social rehabilitation and quality of life of patients with locally-distributed and metastatic bladder cancer by improving the indications and determining the effectiveness of surgical tactics in integrated treatment. It is revealed that the main pathomorphological features that determine the high aggressiveness of tumors and lead to the development of local and metastatic PCM are a significant proportion of low and undifferentiated forms of transition of cellular cancer (57.9%) and a high percentage of squamous cellular (36.8%) and glandular metaplasia (6.1%). As a result of a randomized trial, it has been found that cysticectomy without lymphadenectomy with distributed PCM is a choice operation. This approach compared with PCT significantly improves the quality of life of patients by reducing the symptoms of pain and overall patient health and increasing the mental health of patients, but does not affect its overall duration (p = p = 0.71). The comparative analysis of external urine derivation according to Brickker and modified Ureterocutaneostomy showed the high efficiency of the latter, namely: ease of implementation, reduction of the number of postoperative complications of the early postoperative period by 2.3 times and reduction of the postoperative bed of the day by 2.7 (p <0.01). Ureterocutaneostomy is a simple, less traumatic and highly effective method developed at the clinic: its time of making (187 ± 73) minutes, does not require resection of the intestine, which reduces the level of postoperative complications to 11.7%.
