It has been shown that vitamin D deficiency or deficiency in infancy is observed in 72-90% of healthy children and may be one of the causes of repeated viral infections, and the immune modulating effect of vitamin D occurs through the promotion of the co-operation of the components of congenital and acquired immunity. Vitamin D-dependent regulation of synthesis of HBD-2, SIgA, IL-4, IL-10, lysozyme in children with NDST is established. In 81% of children with RH, changes in BCC and correlations between vitamin D content and indicators reflecting the regulation of electrophysiological function of the myocardium have been detected. In 70% of patients with rheumatoid arthritis, its insufficiency / deficiency was associated with high activity of the disease by ESR, PSA, CIC, HLA-DR + / CD3 + and CD3 + CD4 + -cell, TNF. In chronic hepatitis, 73.3% of patients had deficiency / deficiency of vitamin D, more likely - with autoimmune hepatitis, whereas CD14lowCD16 + monocytes were more often detected in viral hepatitis and had a projective function for liver fibrosis. The concentration of vitamin D in serum depended on the activity of the disease and the stage of fibrosis. Detected genetic predictors of the progressing flow of HCG (allele +3725 C gene TLR4); HGV (APRI> 0.8, vitamin D level <20 ng / ml, allele C TagI of the VDR gene, allele of the +3725 C of the TLR4 gene). It has been established that the displacement wave elastography can be used as a non-invasive diagnostic method for the determination of liver fibrosis less than F3, for application of 2 methods (APRI and elastography) to determine the stage less than F2. The novelty and relevance of the study is to establish the peculiarities of systemic multiparametric relationships between the components of the state of health, the immune system and fibrogenesis, depending on the concentration of vitamin D, which is promising for the development of differentiated pharmacological support schemes on this basis. The scientific significance of the work is to clarify the individual links of pathogenesis and to substantiate the expediency of correcting vitamin D status in healthy children, children with NDST, RH and chronic hepatitis.
