Purpose: to develop personalized approaches to the diagnosis and correction of hormonal and metabolic disorders in patients with diabetes mellitus, depending on phenotypic features, and to substantiate appropriate approaches to treatment. Methods: clinical, laboratory, statistical. Theoretical and practical results. It was established that with an increase in the index of insulin resistance, an increase in the mass of the body, an increase in the waist circumference in patients with DM, there was a decrease in the content of osteocalcin, indicating a slowing down of bone tissue remodeling processes. The degree of obesity in patients with diabetes mellitus is a determining factor in the effect on the activity of APF in men, and the type of obesity - in women. The use of dapagliflozin in patients with elevated levels of uric acid (UA) in serum led to a decrease in uricemia due to increased clearance and fractional excretion of UA. The effect of therapy most clearly manifested in patients with a high degree of obesity, against the background of weight loss and BMW. Treatment with dapagliflozin in patients with high visceral obesity index (VOI) resulted in improved serum lipid profile and decreased uricemia and systolic blood pressure against the background of decreased levels of glycemia and BMI. Therapy with sitagliptin, liraglutide and dapagliflozine improved lipid profile of serum, parameters of body compositions and decreased VOI. It has been established that in the presence of adipose-visceral phenotype in patients, the manifestations of negative psychosomatics are exacerbated. In patients with obesity there was a violation of microcirculation in the blood vessels of the kidneys. The obtained data will facilitate the development of scientifically grounded personalized diagnostic and therapeutic methods for diabetes mellitus. Effectiveness of the incident: improving the diagnosis and treatment of diabetes mellitus in patients with phenotype taking into account.
