The aim of the study: to increase the effectiveness of radiotherapy for genital, head and neck cancer by optimizing topometric preparation using in vivo dosimetry to ensure the quality of radiotherapy. Instrumental methods and equipment: X-ray, morphological, clinical-instrumental, statistical, gamma-therapeutic device ROKUS-AM with Со-60 source (Egamma = 1,25 MV), linear accelerator "Clinac-600C", universal clinical dosimeter "UNIDOSE-E", planning system Eclipse 8.9, X-ray computer tomograph "Toshiba Aquilion 16", simulator "Acuity". Theoretical and practical results: approaches to the correction of the radiation therapy plan were proposed depending on the constitutional features of patients, the type of irradiation and the localization of irradiation fields. It has been proved that repeated planning in patients with oncogynecological pathology reduces the median deviation of the dose from 0.14 to 0.05 %, which constituted of 64.3 %. Scientific data on the incidence and nature of radiation reactions of organs that are a part of the radiation zone under conventional and modified methods of conformal radiation therapy were supplemented. The technique of pre-radiation preparation to the irradiation of tumors of the head and neck with the use of a tongue fixator while taking into consideration the artifacts on tomographic images has been developed. Novelty: the regularity of deviation between the planned dose and the results of in vivo dosimetry was determined depending on the constitutional features, the type of irradiation, the localization of irradiation fields; the influence on critical organs and quality of life of patients was estimated. Efficiency of implementation: reduction of the period of hospitalization for 3-7 days and the cost of treatment of radiation reactions over 3000 UAH.
