Registration Date 26-12-2019 Organization Bogomolets National Medical University, Ministry of Health of Ukraine popup.description2  The relationship between chronic kidney disease in patients (CKD) with acute decompensated heart failure (ADHF) with the greater severity of individual symptoms and signs of congestion, as well as their integral indices of Lala and Gheorghiade and expressiveness and severity of hemodynamical congestion assessed by E/e' and NTproBNP, were established and analyzed. The frequency of worsening of renal function (WRF) at ADHF was established. For the first time, data on the clinical significance of different phenotypes of acute kidney injury in patients with ADHF were obtained. It is shown that in so-called "wet and warm" patients with ADHD WRF is associated with an increase in the incidence of severe fatal and non-fatal cardiovascular complications in the hospital and a greater severity of symptoms only in association with an increase in NGAL after 48 hours, and that WRF is associated with more pronounced clinical-hemodynamic signs of congestion only in patients with reduced glomerular filtration rate (GFR), whereas in patients with preserved GFR this is not observed. It has been shown for the first time that the "nitrate-centric" strategy of eliminating venous congestion in ADHF in patients with CKD has a significant advantage over the "diuretic-centric" strategy in terms of both clinical features of fluid congestion and a safe positive effect on renal function.
