The purpose of the study: considering the current pathomorphosis of mental disorders, to research the features of development, clinical structure, and dynamics in pharmacoresistant forms of mixed affective disorders on the example of bipolar and schizoaffective disorders, to develop scientifically justified schemes for their treatment and prevention of exacerbations. Methods: clinical-psychopathological and clinical-anamnestic method, clinical-follow-up analysis, neuropsychological (study of cognitive functions), methods of mathematical statistics. As a result of research, the peculiarities of the beginning and course of treatment-resistant forms of bipolar and schizoaffective disorders, the main pharmacoresistant clinical variants (dysphoric mania, depression with motor and ideational arousal and ironic depression) and types of incomplete remission (psychopath-like, thymopathic syndromes and residual delusions syndrome) were found. It is advisable prescribing psychopharmacotherapy to consider variants of clinical manifestations and remission types in these disorders. The cognitive functioning patterns in these patients were investigated. The identification of psychopathological and pathopsychological factors influencing the development of resistance allows to prescribe adequate therapy for suicide prevention, occupational and social decline, and the committing of social dangerous activity in patients with bipolar and schizoaffective disorders. Draft protocols of scientifically substantiated treatment and medical care for bipolar and schizoaffective disorders in the conditions of insurance medicine in Ukraine were proposed.
