The purpose of the work is to investigate the morphofunctional foundations of cardiac output formation in myocardial hypertrophy and left ventricular dilatation in various heart pathologies based on the modern theory of myocardial structure. Research methods - morphological (light-optical microscopy, electron microscopy, morphometric research), echocardiography (traditional and speckle tracking), statistical. Morphofunctional characteristics of cardiomyocytes (CMC) in diffuse and/or local systolic and diastolic overload of left ventricular myocardium, mechanisms of adaptation, as well as criteria of maladaptation were studied. It has been proven that CMC hypertrophy is a compensatory response to increasing their function. It has been shown that an increase in systolic load on CMCs causes an expansion of their transverse dimensions due to an increase in the number of myofibrils. It has been found that systolic (transverse) myocardial hypertrophy can be diffuse with increased resistance to cardiac output and local in various types of fibrosis and increased angularity of muscle bundles. It was found that diastolic (longitudinal) myocardial hypertrophy develops in response to overload of the heart cavities with blood volume and manifests itself by lengthening the CMC due to an increase in the number of sarcomers. Decompensation of longitudinal CMC hypertrophy is manifested by desynchronization of their contractions due to myofibril ruptures, as well as myocytolysis due to capillary blood flow deficiency (capillary length does not correspond to CMC length). Longitudinal myocardial hypertrophy in NdMK is accompanied by an increase in semi-long displacement, deformation and rotation in most segments of the basal and middle LSh.
