Purpose: to improve the outcome of LRT with diagnostic and treatment precision technologies considering the prognostic and pathogenetic factors. Methods and equipment: clinical-instrumental, laboratory, enzyme-linked immunosorbent, morphological, radiological, bacteriological, statistical, photodynamic therapy device "Barva-LED / 630", electrodermatometer "Skin observed system", microplate photometer SUNRISE TECAN, ultrasound device "Toshiba". Theoretical and practical results: implementation of the novel algorithm of personalised surgical treatment of LRT based on an assessment of tissue viability in the area of local radiation damage (LRD). The tissue viability index L = 1–0.91– showed satisfactory plastic results, and if L less than 0.9 the tissue can be considered non-viable. Scientific data on the dependence of the occurrence and severity of LRT on follow-up parameters can be used for their prediction. The use of immunocorrective melatonin therapy for the prevention of late toxicity is substantiated, the optimization of immune and hematological parameters is shown. Novelty: a program of measures for conservative treatment of LRT has been developed and includes photodynamic therapy, injections of platelet-enriched plasma. Its effectiveness in 99% of cases is the complete elimination of microorganisms, ulcer healing in 52 days. Criteria for the use of surgical treatment and its combination with various elements of conservative therapy are determined. Implementation efficiency: proved the effectiveness of such treatment in 90% cases, reducing the number of hospitalizations up to 4 times compared to traditional.
