Purpose of the work: to develop criteria for selection of surgical resection or radiofrequency ablation of metastatic liver lesions in colorectal cancer based on the selection of immune and biochemical predictors of the systemic inflammatory response. Methods and equipment: immunological, hematological, biochemical, clinical, morphological, statistical methods; light microscopes, flow cytofluorimeter, spectrophotometer, hematological, and immune enzyme analyzers. Theoretical and practical results: immune and hematological indicators of inflammatory response after radiofrequency ablation (RFA) of colorectal cancer liver metastases were established. On day 3 after RFA, there was an increase in leukocyte, N/L index, decrease in hemoglobin level and eosinophil number, imbalance of CD4+- and CD8 +-lymphocytes, decrease in NK- and NKT-cells number and expression of activating NK-cells receptor NKp46, increase in CD3+HLA-Dr+-, CD4+PD1+-, CD8+PD1+-cells percentage, CRP, IL-6, ferritin levels, and index HOMA-IR. On day 14 after RFA, most of the indicators were close to those defined before RFA, except for the preservation of reduced hemoglobin levels, NK and NKT-cells number, and increased platelet level. Comparative analysis with data obtained after liver metastases resection revealed a lower severity and duration of inflammatory response in RFA. Novelty: changes in the subpopulation composition of lymphocytes and the expression of functional activity markers of T- and NK-cells in the inflammatory response caused by RFA, which may be involved in pathogenetic mechanisms that contribute to cancer progression, were established. Implementation efficiency: determining the nature of immune disorders provides criteria for prescribing adequate perioperative therapy to improve cancer prognosis.
