The aim is to increase the effectiveness of treatment of patients with pulmonary tuberculosis (TB) by determining objective indications for surgery based on computer densitometric characteristics of the disease and improving the technique of operations. Methods and equipment – clinical, surgical, radiological, laboratory; videothoracoscopic complex "Contact", ventilators "Dragger", "Angstrom"; electric welding complex EK-300M1. It is established that the values of densitometry, from 15 HU to 55 HU indicate the relative, below 15 HU – the absolute indications for surgery, over 55 HU – the feasibility of the operation is decided individually.It is determined that at increased (over -730 HU) density of the parenchyma observed obliteration of the pleural cavity and interlobular fissures, fibrous changes in the lung root, at reduced density (below -860 HU) – bleeding and insufficient hermeticity of the parenchyma suture, which together aggravated the early postoperative period and contributed to the overall unsatisfactory results. Tightness of the separated pulmonary parenchyma and, as a result, reduction of term of postoperative stay in a hospital from 20,7 days to 14,5 days. An algorithm for the application of surgical treatment of patients with pulmonary tuberculosis using computer densitometry, which provides a reasonable sequence of actions for the formation of a contingent of patients with indications for surgery, determining the timing, type and technique of surgery, which allowed to cure 97,0 % of patients with a reduction in the number of relapses to 3,3 % and no mortality.
