Aim – to develop the methods of prophylaxis and principles of personalized therapy of pulmonary sarcoidosis relapses. Methods – clinical, computed tomography of chest, pulmonary ventilation and diffusion diagnostic tests. In patients after long-term glucocorticosteroid (GCS) therapy the rate of relapses (43,2 %) was many times higher than in patients with spontaneous remission (3,7 %). Pulmonary sarcoidosis relapses are observed less frequently after successful complication of therapy using methotrexate (MTX), comparing with methylprednisolone. There was created a technology of predicting the relapses rate of sarcoidosis depending on the therapy, that helps to determine the number and frequency of clinical visits of the patients during the follow-up. It was established that the number of clinical cure without sequalae cases has significantly increased in methotrexate 15 mg/week arm comparing to 10 mg/week arm (81,3 % and 42,4 %, respectively). The principle of sarcoidosis relapse therapy is based on the prescription of immunosuppressive medication. There was developed a method of treatment of the release with MTX, decreasing the rate of relapse by 2,6 times and the incidence of adverse reactions by 1,5 times in comparison GCS therapy.There was developed a clinical model of phenotype of relapsing pulmonary sarcoidosis. There were developed the methods of prophylaxis of pulmonary sarcoidosis relapse based on the use of MTX 15 mg weekly as initial therapy in patients with relapsing sarcoidosis phenotype or 10 mg weekly in patients without risk factors of the disease relapse.
