The purpose of the work is to increase the efficiency of surgical treatment of patients with pulmonary and pleural tuberculosis. Research methods - clinical-instrumental, laboratory, cytological, microbiological, pathomorphological, statistical. The optimal time for minimally invasive surgery in patients with pulmonary TB is a period of 2 months. (for sensitive TV) and 4-6 months. from the beginning of anti-tuberculosis therapy (АTT) (for patients with resistant forms of TB). Minimally invasive operations were shorter than the average duration of surgery – (75.1 ± 22.3) min, intraoperative blood loss – (85.4 ± 1.6) ml, and the postoperative period was characterized by a low level of intraoperative (3.8 ± 1.7) % and postoperative complications (13.1 ± 3.0) %. More often video-assisted operations were performed on pulmonary tuberculomas – 68.8 %, more with moderate activity of the process – 37.5 %. 8 new medical technologies were developed, which allowed to reduce the level of complications from 6.6 % to 14.5 %, A new method of video-assisted lung resection in patients with multidrug-resistant tuberculosis has been developed, the use of which has reduced the incidence of complications by 6.6%; reduce the patient's stay in the hospital by 5.6 days; increase the overall effectiveness of treatment by 6.6%. An algorithm for choosing the method of surgical intervention was developed, which allowed to determine the sequence of actions of the doctor, to establish the procedure for the application of diagnostic and therapeutic measures and methods for assessing the results of treatment.
