Purpose: to develop criteria for choosing surgical resection or radiofrequency ablation of metastatic liver lesions in colorectal cancer based on the selection of immune and biochemical predictors of the systemic inflammatory response. Methods and equipment: immunological, hematological, biochemical, clinical, morphological, statistical methods; light microscopes, flow cytofluorimeter, spectrophotometer, hematological, and immune enzyme analyzers. Theoretical and practical results: immune and hematological indicators of the inflammatory response after radiofrequency ablation (RFA) of colorectal cancer metastases in the liver were established. 3 days after RFA, an increase in the number of leukocytes, the H/L index, a decrease in the level of hemoglobin and the number of eosinophils, a violation of the balance of CD4+ and CD8+ lymphocytes, a decrease in the number of NK and NKT cells and the expression of the activating NK cell receptor NKp46, an increase in the percentage of CD3 +HLA-Dr+-, CD4+ PD1+-, CD8+PD1+-cells, levels of CRP, IL-6, ferritin and HOMA-IR index. On day of 14, most of the indicators were approached to the RF, except for the preservation of reduced hemoglobin levels, NK- and NKT cells and increased platelet levels. Comparative analysis with the data obtained after resection of metastases in the liver revealed less than the severity and duration of the inflammatory response during RFA. Novelty: the complex of indicators on the basis of the subpopulation composition of lymphocytes, markers of functional activity of T- and NK cells and biochemical markers of inflammatory reaction caused by RCA or surgical treatment, which are involved in pathogenetic mechanisms that contribute to the progression of cancer. Implementation efficiency: improving the effectiveness of treatment.
