Yanovsky National academy of medical sciences of Ukraine" popup.description2 The object of the study–90 patients with pleural empyema of with bronchopleural fistula.The aim–to increase the effectiveness of treatment of patients with tuberculosis and non-specific lung diseases with accompanying pathology using objective indications for the use of valvular bronchoblockage.Methods and equipment–clinical,surgical,radiological,laboratory; multiscan computer tomography, video thoracoscopic complex, video fibrobronchoscopic complex, endobronchial bronchial occluder, bed-side pleuroaspirator.It has been established that the main indicators for carrying out endobronchial occlusion and planning its stages are the presence of clinical signs of broncho-pleural connection, radiological characteristics of the remaining cavity, among which undemarcated, demarcated, multi-chambered cavities with a content of up to 10 HU and more than 10 HU are distinguished, the state of the bronchial tree in relation to of an inflammatory process, in which local endobronchitis at the site of valve placement does not exceed the 1st degree of inflammation, the allocation of gradations in the parenchyma (the presence of fibrosis below–700 HU or emphysematosis above–900 HU relative to the visually unaffected parenchyma, infiltration within the range≥ –30 HU–≤ 30 HU or destructions< –900 HU vith visualy detected pathologic changes) for planning the volume and sequence of treatment; clinical, bronchological and radiological signs of recovery, the absence of focal changes with a density of more than 30 HU in the lung parenchyma for the termination of endobronchial occlusion.The development method of treatment of patients with bronchopleural connection, which allows to ensure personalized planning and treatment based on objective data, prevention of unnecessary surgical interventions or manipulations, reduction of the length of stay in the hospital,on average,by 24,6%, complete cure of all patients without using any additional surgical interventions.Field of application – surgery.
